When tattoo pigment is injected into the dermis, the body's immune system immediately recognizes it as a foreign invader. Specialized cells attempt to remove the ink particles, but they are too large. This sets up a permanent state of low-grade inflammation as the immune system continuously fights a battle it cannot win.

The tattooing process also breaks the skin's protective barrier, raising risks for infections like staph, hepatitis, and mycobacterial infections.
