An official investigation has concluded that the Vancouver Police Department failed Tatyanna Harrison, a woman who went missing in 2022. Her remains were discovered on a drydocked yacht in Richmond in May 2022, though identification was not confirmed until August.

Two Vancouver police officers were found to have committed misconduct for neglecting to conduct a missing person's risk assessment during the initial investigation. A third officer acknowledged misconduct in the case.

These failures occurred despite policing standards requiring immediate risk assessments in missing persons cases. The investigation also raised questions about the Vancouver police's internal risk assessment template, noting it uses different criteria than the provincial standard.

The findings come years after the Missing Women Commission of Inquiry, established after the Pickton case, aimed to improve handling of missing persons cases.

The BC Coroner's Office has called for an inquest into Harrison's death, though no date has been set.
