Interim results from the Phase 3 MonumenTAL-3 trial show talquetamab-based combinations dramatically improve outcomes for patients with relapsed or refractory multiple myeloma. The study enrolled 864 patients, comparing talquetamab plus daratumumab and pomalidomide (Tal-DP), or talquetamab plus daratumumab (Tal-D), against a standard DPd regimen.

After a median follow-up of 24.6 months, both talquetamab arms significantly reduced the risk of disease progression or death. The hazard ratio for progression-free survival was 0.28 for Tal-DP and 0.33 for Tal-D. The 24-month progression-free survival rates reached 81.3% and 77.6% respectively, far exceeding the 51.2% observed with DPd.

Overall response rates topped 88% for talquetamab regimens. Complete response or better was achieved in over two-thirds of treated patients. Minimal residual disease-negative complete responses were observed in more than 46% of participants. Safety profiles remained consistent with known agent risks. Cytokine release syndrome and GPRC5D-related effects were predominantly low grade and rarely led to discontinuation.
